Once all the information requested for the referral has been received, arrangements will be made to invite the young person and their allocated social worker to visit the accommodation, and a draft plan is made available for discussion and subsequent agreement. The intention of the meeting will be to share, review and hand-over any relevant information relating to the service user.
Examples of the information we will require are as follows;
Current Pathway plan, minutes from the LAC Review;
Placement plan; Health plan;
Personal Education plan;
Missing and Key Contacts checklist;
Risk assessment. This will enable us to start planning the best possible start for the young person.
