    Types of Supported Housing
    Young Roots Care Services provide a varied range of support and care provisions to young people including:

    Supported Housing

    This service provides accommodation and support services for young people aged 16-21, including those leaving care, unaccompanied asylum seeking children and teenage parents. Following a comprehensive needs and risk assessment and the preparation of a support plan, the objective of the service is the progression of the young person to "move on" and live independently.

    Our in-house key workers focus on matters such as life skills, identifying and sourcing education/training, accessing health and benefits

    Monitored Accommodation

    Monitored accommodation and support services are provided for individuals aged 16 years old onwards, with a variety of needs. The support and accommodation services are usually provided on two basic levels, but are flexible and adaptable to meet the service users needs: Low Support Service: This accommodation is regularly monitored by a floating team. The monitoring team will undertake planned, routine and reactive visits each week, including out of hours and weekends. Each Service User receives up to four hours of 1:1 key work support each week.

    Medium Support Service: This level of service provides a greater and more structured staffing presence within the accommodation, including in the evening and weekends. Each Service User usually receives a minimum of seven hours of 1:1 key work support each week.

    Floating Support

    Floating support is available and provided to eligible young people, as required and agreed. Our in house Key Workers work to meet the young person's needs and achieve positive outcomes, with the overall aim to progress the young person to "move on" and live independently.
